Why vaccination immunity fades over time for some diseases

Getting vaccinated is often treated as a one-time box to tick, yet for several diseases, the protection a vaccine provides does not last forever. This is not a flaw in the vaccine itself but a reflection of how the immune system naturally works, and it explains why booster doses exist for some illnesses and not others.

How immunity is built in the first place

When a vaccine is administered, it introduces a harmless piece or weakened form of a pathogen to the body. This trains the immune system to recognise it and produce antibodies, along with memory cells that remain on standby in case of future exposure.

 According to the World Health Organization (WHO), this process is what allows the body to respond quickly and effectively if it ever encounters the real disease, often preventing severe illness altogether.

Why the protection doesn’t always last

Not all immune responses are built the same way. Some vaccines trigger a strong, long-lasting memory response, while others produce a weaker one that fades with time. The Centers for Disease Control and Prevention (CDC) notes that antibody levels naturally decline after vaccination, and for some diseases, this decline happens fast enough that protection drops before the memory cells can be relied upon alone.

Whooping cough and tetanus are commonly cited examples where immunity wanes over years, which is why booster shots are recommended periodically rather than treated as optional.

The type of pathogen also plays a role. Viruses and bacteria that mutate frequently, such as influenza, require updated vaccines almost every year because the version circulating changes, not necessarily because old immunity has vanished entirely.

Age and health also matter

The immune system does not respond the same way throughout life. The WHO has noted that immune function tends to weaken with age, a process sometimes referred to as immunosenescence, which can cause vaccine-induced protection to fade faster in older adults compared to younger, healthier individuals. 

This is part of the reasoning behind offering certain vaccines, such as those for pneumonia or shingles, specifically to older populations.

Underlying health conditions and a weakened immune system, whether from illness or certain medications, can also affect how long protection lasts, sometimes making booster doses necessary sooner than for the general population.

Why boosters exist

Rather than signalling that a vaccine failed, the need for a booster reflects how the body’s memory response naturally works for that specific disease. The Kenya Ministry of Health has continued to encourage completion of full vaccination schedules, including booster doses where recommended, as a way of maintaining protection rather than assuming a single dose offers permanent immunity.

Understanding that immunity can fade is not a reason to distrust vaccines. It is simply a reminder that, for some diseases, staying protected is an ongoing process rather than a single event.
