Why some people hear ringing with no sound around them

Sometimes, a person may be sitting in a quiet room when a ringing, buzzing, or humming sound suddenly seems to come from their ears. There may be no phone ringing, appliance running, or other obvious source of noise, yet the sound feels real.

This experience is known as tinnitus. It is the perception of a sound that does not come from an outside source, meaning other people may not hear what the affected person is hearing. According to the National Institute on Deafness and Other Communication Disorders (NIDCD), tinnitus can sound like ringing, buzzing, roaring, whistling, hissing, or humming. It can occur in one ear, both ears, or seem to come from inside the head.

What causes the ringing?

Tinnitus is often linked to changes in the hearing system. One common factor is exposure to loud noise, such as loud music, machinery, concerts, or prolonged use of headphones at high volume.

Repeated exposure to loud sounds can damage sensitive cells in the inner ear. Even when a person feels that their hearing has returned to normal after a loud event, there can still be damage that may later contribute to hearing problems and tinnitus.

Hearing loss associated with ageing is another common factor. The brain relies on signals from the ears to process sound. When those signals change because of hearing loss or damage to the inner ear, the brain’s sound-processing system may also change, which can contribute to the perception of sounds that are not actually present.

It is not always caused by hearing loss

Although hearing problems are strongly associated with tinnitus, they are not the only possible causes.

A build-up of earwax or an ear infection can sometimes trigger ringing by affecting the way sound reaches the ear. Certain medicines have also been linked to tinnitus, including some antibiotics, anti-inflammatory medicines and other drugs.

Other conditions, including Ménière’s disease, diabetes and thyroid disorders, have also been associated with tinnitus. In some cases, however, doctors may not be able to identify a clear cause.

The sound can also become more noticeable when the surroundings are very quiet. This is one reason some people notice ringing more when they are trying to sleep or sitting alone in a silent room.

When should you seek medical attention?

Occasional ringing that disappears on its own may not necessarily mean there is a serious problem. However, persistent or worsening tinnitus should not simply be ignored.

According to the NHS, people should seek medical advice if tinnitus occurs regularly or constantly, becomes worse, or starts affecting sleep, concentration or emotional wellbeing. A doctor may examine the ears for problems such as earwax or infection and may also check for hearing loss.

Particular attention is needed when the sound seems to beat in time with the person’s heartbeat. This type of tinnitus can sometimes be related to blood-flow problems and should be assessed by a healthcare professional.

Tinnitus accompanied by sudden hearing loss, severe dizziness, facial weakness or symptoms following a head injury also requires urgent medical attention.

Protecting your hearing

While not every case of tinnitus can be prevented, protecting the ears from excessive noise can reduce the risk. Lowering the volume of headphones, moving away from very loud sounds, and using appropriate hearing protection in noisy environments can help protect hearing.

For people already experiencing tinnitus, treatment depends on the possible cause and how much it affects daily life. Addressing problems such as earwax, infections or hearing loss may help, while sound-based approaches and counselling can help some people manage persistent symptoms.

The ringing may sound as though it is coming from somewhere around you, but tinnitus is actually linked to the way the hearing system and brain process sound. Understanding this can help explain why a person can hear a very real sound even when the room around them is completely quiet.
